There is an immediate shortage of Midazolam 5 mg/ 1 ml concentration. Once you use yours, you will not be able to replace it. We are currently working on plan B.
A few important things to remember: You may use the 5 mg / 5 ml concentration for IM injections, but you will need to split the dose as 5 ml is too much volume. You may NOT use the 5 mg/5 ml concentration for IN administration UNLESS your calculated dose is 2mg or less (1 ml maximum each nare)
When possible, please utilize your 5 mg/ 5 ml concentration via IV.
Once you use your 5:1 concentration – do NOT replace it with another 5:5 vial. (You will go down to 15 mg total on your unit, and that is fine.) Please notify me when the 5:1 vial has been used on your unit so I can keep track of how many will need replaced once the supply returns.
Please note: there is no stock of 5:1 in pyxis. If you have to waste, you will need to complete the waste documentation in the appropriate section of your emeds report as well as your narcotics log.
I encourage you to look over your protocols to review your dosages in light of this lack of concentration.
