All clinicians,
The mandatory reporting process for Child Protective Services has been updated. Per COMAR, upon recognition of a case that is required to be reported, you will need to complete ALL of the following:
1. Call the hotline. 1-800-91PREVENT (1-800-917-7383) Please update all your personal and unit phones to include this number. 2. Complete a MD 180 form. These can be found at: chrome-extension://efaidnbmnnnibpcajpcglclefindmkaj/https://dhs.maryland.gov/documents/Child%20Protective%20Services/180%20Form%20with%20instructions-fillable.pdf 3. You will need to send that completed MD180 form to two locations: * Email [email protected] or fax 240-420-2549 AND * Email [email protected]<mailto:[email protected]> or fax 240-313-2001
I have attached a copy of the fillable form, as well as a helpful handout from the Department of Human Services with frequently asked questions regarding making a report for suspected child welfare/abuse/neglect. The handout and form has a lot more detail.
As always, make sure to document the referral in the narrative portion of your patient care chart in EMEDS.
