Please take a few moments today to review the protocols for Ketamine dosages for pain management. Review the concentrations carried in your units, and refamiliarize yourself with the 10 mg/ml concentration (versus the 50 mg/1 ml concentration normally used for IM/IN doses as well as excited delirium).
It is very important that you select the correct concentration (10 mg/cc) for IV use and use the appropriate dose of 0.2 mg/kg (IV/IO). We have had several unintentional IV overdoses of ketamine in the last month for pain management. Maximum single dose is 20 mg.

I did not add the IM/IN dosages (0.5mg/kg) to this chart, as the current ketamine errors have been with IV administration. I will be glad to add them upon request. Make sure you review your entire ketamine protocol to ensure your working knowledge of the drug to include indications, contraindications, adverse effects, and precautions.
Remember ANY sedation doses of ketamine administered to patients require a second ALS provider.
